Walk-On Forms and Information
The following is a list of information you must present to the athletic training staff before you will be permitted to try out for a varsity athletic team.

• Proof of a physical within in the last 12 months
• Physical must include date and physician signature
• Both release of claims forms
• If you are under 18, forms must be co-signed by a parent or guardian
